Shortness of breath is a symptom that many people experience at some point, whether after physical activity, during an illness, or as part of getting older. What Is COPD? 

Chronic obstructive pulmonary disease, commonly called COPD, is a group of lung diseases that make it harder to move air in and out of the lungs. 

The two most common forms of COPD are chronic bronchitis and emphysema. Both conditions can cause long-term inflammation and damage within the airways and lung tissue. 

COPD is most commonly associated with smoking, although long-term exposure to certain environmental irritants and other factors can also contribute. 

The Centers for Disease Control and Prevention states that COPD is a major cause of chronic illness and can significantly affect a person’s ability to breathe and remain physically active. 

What Does COPD Shortness of Breath Feel Like?

When Shortness of Breath Occurs

COPD-related shortness of breath often:

Shortness of breath is often the symptom people notice first, but other changes may help identify whether the issue is related to the heart or lungs. 

Heart failure may be associated with: 

  • Swelling in the legs, ankles, or feet 
  • Sudden weight gain from fluid buildup 
  • Fatigue and weakness 
  • A rapid or irregular heartbeat 
  • Reduced exercise tolerance 

COPD may be associated with: 

  • Persistent coughing 
  • Wheezing 
  • Increased mucus production 
  • Frequent respiratory infections 
  • Chest tightness 

Recognizing these additional symptoms can help healthcare providers determine what tests may be needed. 

Why Heart Failure and COPD Are Sometimes Confused 

The heart and lungs work together to deliver oxygen throughout the body, which is why problems in one system can affect the other. 

For example, a person with COPD may experience strain on the heart because the lungs are not delivering oxygen efficiently. Over time, this can contribute to changes in the heart and circulation. 

Likewise, a person with heart failure may feel like they have a lung problem because fluid buildup can make breathing more difficult. 

This overlap is one reason shortness of breath should not be self-diagnosed. A symptom that appears to be related to breathing may require a cardiovascular evaluation to determine whether the heart is contributing. 

Risk Factors For Heart Failure 

Heart failure can develop when conditions weaken or damage the heart over time.

  • Certain heart rhythm disorders

Risk Factors For COPD 

COPD is most commonly associated with long-term exposure to substances that damage the lungs. 

Risk factors include: 

  • Smoking or a history of smoking 
  • Secondhand smoke exposure 
  • Workplace exposure to dust, fumes, or chemicals 
  • Long-term air pollution exposure 
  • Certain genetic conditions 

While smoking remains the leading risk factor, COPD can affect people with other types of long-term lung irritation as well. 

When Should Shortness of Breath Be Evaluated? 

Occasional breathlessness after intense activity is normal. However, shortness of breath that is new, worsening, or affecting everyday activities should be evaluated. 

You should talk with a healthcare provider if you experience: 

  • Shortness of breath with normal activities 
  • Increasing difficulty walking or exercising 
  • Breathing problems that worsen over time 
  • Chest discomfort 
  • Swelling in the legs or feet 
  • Unexplained fatigue 
  • Rapid weight changes 

It is especially important to seek medical attention if breathing problems occur suddenly or are accompanied by chest pain, fainting, severe weakness, or confusion. 

Identifying whether symptoms are related to heart failure, COPD, or another condition is the first step toward receiving appropriate care. 

How Heart Failure and COPD Are Diagnosed 

Because heart failure and COPD can produce similar symptoms, determining the cause of shortness of breath often requires a thorough evaluation. 

A healthcare provider may review your symptoms, medical history, risk factors, and perform testing to better understand what is affecting your breathing. 

Diagnostic testing may include: 

  • Echocardiogram to evaluate heart structure and function 
  • Electrocardiogram (EKG) to assess heart rhythm 
  • Chest imaging 
  • Lung function tests 
  • Blood tests 
  • Exercise or stress testing when appropriate 

These evaluations help identify whether symptoms are related to heart function, lung function, or a combination of both. 

Treatment Depends on the Cause 

Although heart failure and COPD can feel similar, they require different treatment approaches. 

Heart failure treatment focuses on improving heart function, managing fluid levels, and reducing strain on the cardiovascular system. Depending on the individual, treatment may include medications, lifestyle changes, monitoring, and procedures designed to improve heart health. 

COPD treatment focuses on improving airflow, reducing symptoms, and preventing flare-ups. Treatment may include inhaled medications, pulmonary rehabilitation, oxygen therapy when needed, and lifestyle changes. 

For some patients, heart and lung conditions may exist together. When that happens, coordinated care is especially important to address all factors contributing to shortness of breath. 

Do Not Ignore Changes in Your Breathing 

Shortness of breath is easy to explain away. Many people assume they are simply out of shape, getting older, or dealing with a temporary issue. 

However, changes in breathing can be an important signal from your body. 

Heart failure and COPD are different conditions, but both can affect your ability to complete everyday activities and maintain your quality of life. Recognizing symptoms early can lead to earlier diagnosis, better symptom management, and more effective treatment.
